At Grand Itasca, excellence isn't just a goal; it's a commitment woven into the fabric of our care. We are proud to announce that Grand Itasca's interdisciplinary team has received the 2024 Partners with Distinction award from Fulcrum Health for their groundbreaking work in implementing the Spine Therapy Program. This prestigious award recognizes health care providers who epitomize patient-centered care and demonstrate exceptional team-based approaches.
Fulcrum Health held its annual symposium and Partners with Distinction awards celebration last October in Minneapolis, where Dr. Jay Sonder, who spearheaded the creation of this program, accepted the award in the Bridging the Gap of Team-Based Care category.
The Spine Therapy Program: A Beacon of Patient-Centered Care
Our Spine Therapy Program stands as a testament to our dedication to providing holistic and personalized care to individuals suffering from chronic neck and lower back pain. Dr. Jay Sonder spearheaded the creation of this program, focusing on tailoring treatment plans to meet each patient's unique needs. Our team of providers ensures that every individual receives the highest quality care through comprehensive evaluations and collaborative efforts.
A Multidisciplinary Approach to Healing
Our multidisciplinary team-based approach is central to the success of our Spine Therapy Program. Since its inception in 2019, the program has brought together diverse health care professionals, from disciplines including occupational medicine, chiropractic care, physical therapy, and acupuncture. “This integrated approach allows us to address the multifaceted aspects of spine health and deliver comprehensive treatment solutions,” said Sonder.
Pioneering the Future of Health Care
“As we celebrate the continued success of our Spine Therapy Program, I envision a future where integrated care settings become the norm and health care providers serve as the architects of transformative care programs,” says Sonder. With our proven track record of success and commitment to innovation, we are leading the charge toward a more integrated and patient-centric health care landscape.
